入会資格 / Eligibility for Membership
（１）トライフォース大島の会則を承認し、諸規則を遵守する方。
（１）Those who accept the rules of Triforce Ojima and abide by the regulations.
（２）暴力団・暴力団員その他これに準ずる者等反社会的勢力でない方。また、将来にわたりこれに該当しないことを自ら保証する方。
（２）Those who are not members of anti-social forces such as gangs, gang members, or other similar groups, and who guarantee that they will not fall into this category in the future.
（３）過去に本スクール、他のスクール、または会員制スポーツクラブ等で除名となったことがない方。過去に除名となったが、会社が別途定める基準に応じて再度入会資格を認めた方。
（３）Those who have never been expelled from this school, other schools, or membership-based sports clubs, etc. Those who have been expelled in the past but have been re-eligibly admitted to membership according to the criteria separately set by the company.
（４）次のいずれにも該当しない方。※該当項目がある方はオンライン入会を利用せず別途Eメールにてご相談下さい。
（４）Those who do not fall into any of the following categories. *If any of the following categories apply to you, please contact us by email instead of joining online.
● 現在、他の柔術スクールに会員として在籍されている方
● Those who are currently enrolled as members of another Jiu-Jitsu school
● 刺青、ファッションタトゥーがある方
● Those with tattoos or fashionable tattoos
● 身体的障害、傷病、高齢などにより施設を一人で利用できない方
● Those who are unable to use the facilities alone due to physical disabilities, injuries, illnesses, advanced age, etc.
● 集団感染するおそれのある疾病（ 感染症・感染性皮膚病）の方
● Those with diseases that may spread in groups (infectious diseases or infectious skin diseases)
● 入会手続きの時点で妊娠している方
● Those who are pregnant at the time of joining
● 狭心症・心筋梗塞・先天性心疾患と診断されたことがあり、医者に運動を控えるように言われている方
● Those who have been diagnosed with angina pectoris, myocardial infarction, or congenital heart disease and have been advised by a doctor to refrain from exercising
● 安静時心電図で異常を指摘された事があり、医者に運動を控えるように言われている方
● Those who have had abnormalities detected in their resting electrocardiogram and have been advised by a doctor to refrain from exercising
● てんかんを起こしたことがあり、医者に運動を控えるように言われている方
● Those who have had epilepsy and have been advised by a doctor to refrain from exercising

免責の同意 / Agreement to Disclaimer
本スクールにおける格闘技の練習プログラムには身体的な危険が伴うことを承知し、本スクールの施設利用、練習、行事、試合等において本人または第三者に怪我、死亡等の人的事故が生じた場合、トライフォース大島は一切損害賠償･補償等の責を負わないことを了承する。
I understand that the martial arts training program at this school involves physical danger, and I understand that Triforce Ojima will not be held liable for any damages or compensation if an accident such as injury or death occurs to me or a third party during the use of the school's facilities, training, events, matches, etc.
ご本人の誓約 / Your pledge*
上記の入会資格をすべて満たしていることを保証し、入会後に入会資格に該当しないことや入会に際して虚偽の申告をしたことが発覚した場合には、その時点で除名となることを了承し、免責内容に同意した上でトライフォースへの入会を申し込み致します。

未成年である入会申込者の親権者として、トライフォースの会則に基づく責任を本人と連帯して負うことを了承した上で、申込者のトライフォースへの入会を申し込み致します。

（１）入会金と会費のお支払いについて
（１）Explanation of important matters
●入会金、及び初期費用として納入する前払いの月会費は、契約締結のためのものであり､理由の如何を問わず会員にこれを返還致しません。
●The admission fee and the monthly membership fee paid in advance as an initial fee are for the purpose of concluding a contract, and will not be refunded to members regardless of the reason.
●月会費の請求業務はすべて集金代行会社（Square 会費ペイ）に委託しており、会費の自動振替を行うためにクレジットカード、銀行口座を登録して頂く必要があります。会費ペイでご登録の場合はシステム利用110円が掛かります。
●All monthly membership fee billing is outsourced to a collection agency (Square Membership Fee Pay), and you will need to register your credit card and bank account to automatically transfer the membership fee. If you register with Membership Fee Pay, a system usage fee of 110 yen will be charged.
●月会費は毎月27日（Square　会費ペイ登録の場合は26日）に翌月分を自動振替いたします。振替予定日の前日までに口座へご入金ください。
●Monthly membership fees will be automatically transferred on the 27th of each month (26th if you are registered with Square Membership Pay). Please deposit the funds into your account by the day before the transfer date. ●月会費の支払いに遅延があった場合、あるいは窓口でのお支払いとなった場合は、所定の手数料（1000円/1ヶ月）を会員にご負担して頂きます。
●If there is a delay in payment of the monthly membership fee, or if payment is made at the counter, the member will be required to pay the designated fee (1,000 yen/month).
●月会費は施設の利用の有無にかかわらず退会月までお支払いが必要です。
●Monthly membership fees must be paid until the month of withdrawal, regardless of whether or not you use the facilities.
●遅延、滞納など月会費のお支払い状況により、会員に通知連絡することなく除名とさせていただく場合がございます。この場合、退会月まで完納いただきますが、処分は取り消しいたしません。
●Depending on the payment status of monthly membership fees, such as delays or defaults, we may expel members without notifying them. In this case, you will be required to pay all fees up until the month of withdrawal, but the expulsion will not be reversed.
●年一括払いは入会初年度のみ可能で、利用希望月の1日から1年間の契約として2ヶ月分の割引が適用されます。期間中に退会されましても、未経過分はご返金出来ません。
● Annual lump sum payment is only available for the first year of membership, and a two-month discount will be applied as a one-year contract from the first day of the month you wish to use the service. Even if you cancel your membership during the period, any unused portion will not be refunded.

（２）休会、退会について
（２）Regarding temporary leave and withdrawal
●休会する場合は希望月の前月12日までにご来館下さい。電話やemailでは受け付けておりません。休会期間中は１ヶ月２０００円の休会費のみを御引き落としさせて頂きます。 ※休会は2ヶ月以上お休みなる場合のみ可能になります。
●If you wish to suspend your membership, please come to the facility by the 12th of the month prior to the month you wish to suspend your membership. We do not accept requests by phone or email. During the suspension period, only the suspension fee of 2,000 yen per month will be deducted from your account. *Suspension is only possible if you are absent for 2 months or more.
●退会する場合は希望月の前月10日までにご来館下さい。電話やemailでは受け付けておりません。未納会費がある場合は来館時にご清算して頂きます。

●If you wish to cancel your membership, please come to the facility by the 10th of the month prior to the month you wish to cancel. Cancellations cannot be made by phone or email. If you have any outstanding membership fees, you will need to settle them when you visit the facility.

（３）各種手続きのご案内

●入会時の諸会費、諸料金は時期、キャンペーン等により変動することがございます。
●Membership fees and other charges at the time of registration may vary depending on the time of year, campaigns, etc.
●トライフォース大島からの重要・緊急なお知らせは、Eメールにて配信されます。郵送によるご連絡は原則として致しません。
●Important and urgent notices from Triforce Ojima will be sent via email. As a general rule, we will not contact you by post.
●トライフォース大島からの郵便によるご案内は、お届け頂いた最新の住所へ発送いたします。ご登録が現住所と異なる場合、郵便物を希望されない場合など、不到達以後の責任を負いません。
●Triforce Ohshima will send notices by post to the most recent address you provided. If the address you registered is different from your current address, or if you do not wish to receive mail, we will not be responsible for non-delivery or any other issues.
●退会、その他の諸手続きは、所定の期日までに所属スクールにご来館頂いてのお手続きが必要となります。口頭、電話、ファックス、郵送、インターネット、メール、代理人によるお手続きは受付いたしておりません。
●For withdrawal and other procedures, you must visit your school by the designated date. We cannot accept procedures submitted verbally, by phone, fax, mail, internet, email, or by proxy.
